Be warned! Here be spoilers ahead for Marvel's 'What If...?' and Loki.

The multiverse is here in the MCU and Doctor Strange is going to play a huge part of dealing with it in his upcoming film Doctor Strange in the Multiverse of Madness, but now fans are talking about his appearance in Spider-Man: No Way Home too. We know for sure that Doctor Strange is key to Phase 4 and that has led to some serious speculation about who the villain may be in future MCU films.
Could it be... Doctor Strange himself?

A fan theory on CBR.com paints a compelling picture that he could be.
Disney+'s What If...? has had quite an impact on the MCU. In season 1 episode 4 of the show, titled, “What If… Doctor Strange Lost His Heart Instead of His Hands?” the show takes on the destruction of a reality by a Doctor Strange variant who went very bad. The episode came out around the same time as the No Way Home trailer, so some people are connecting the two — could this be the major twist for both it and Multiverse of Madness?
The creation of the Multiverse came at the end of the Loki season finale and obviously has huge repercussions we'll see in No Way Home. The teaser showed us Doctor Octopus's reappearance in a crossover from the non-MCU Sony franchise. This reality-twisting calamity is apparently a result of Doctor Strange trying to erase the public's knowledge that Spider-Man is Peter Parker.

The new theory comes from a few key connections in "Lost His Heart." A car accident robs Strange of the use of his hands in the regular MCU, but in the 'What If...?' episode Christine Palmer dies in the accident instead. He tries to bring her back from the dead, but cannot because that would create a paradox in his universe. Obsessed with righting the wrong despite being warned by the Ancient One, Strange absorbs the energy of many supernaturals and finally his own duplicate. Christine is restored but she's horrified by the monstrosity he's become and ultimately his universe is destroyed by the blowback. The episode ends with "Stephen Strange Supreme" alone in a void, tortured by his own actions.

Our heroic Doctor is only a little bit different from our evil one, which is perhaps the most interesting part of the episode. Hero-doctor is, perhaps, only a little less arrogant than evil-doctor, as if that may be the thing that tips one Doctor to another.
In the No Way Home trailer, Strange seems incredibly full of himself and plays with reality confidently—despite being warned by Wong. That's not necessarily out of order for our normal Hero, but it may be an indication that Strange isn't quite... right.

Additionally, one scene from the trailer shows Doctor Strange encircling a surprised Spider-man with a series of train cars in what might be a fight. Another scene depicts the Sorcerer Supreme pushing Peter Parker's soul from Spider-man's body as Strange's voiceover blames him for "trying to live two lives."
"Lost His Heart" plays on that idea; could we be seeing an alternate Doctor Strange? Only time will tell.
